Our discussion topic is inspired by a recent posting on the Idisaster 2.0 Blog titled "Social media during crisis response: Five general lessons for emergency managers." In order to stimulate the discussion, ten questions have been posted and participants are encouraged to review them in advance.

Our guest will be Kim Stephens, a researcher with a special focus on the application of social media to the practice of emergency management, and author of the blog post. Kim has over a decade of experience in the field of emergency management, both as a researcher and practitioner including federal, local and non-governmental organizations. She is currently developing social media training for the 2011 Readiness in Emergency Management for Schools (REMS) grantees.

The Hospital Surge Model

The Hospital Surge Model now allows users to estimate the resources needed to respond to emergencies involving improvised explosive devices, pneumonic plague and foodborne botulism. The Web-based interactive model now includes a total of 13 scenarios on biological incidents and attacks ranging from pandemic influenza to a nuclear explosion.

Hospital and emergency planners can use the Hospital Surge Model to develop specific strategies to treat an influx of patients affected by these specific incidents. It will estimate, by day, the severity of injury and the number and flow of casualties needing medical attention for specific scenarios selected by users. Hospital Fire Safety Conference 2010

One of the more challenging issues facing a fire service is receiving a call about an active fire at a healthcare facility. Regardless of the cause, the results can be devastating. Preparing for and responding to a hospital fire is a very unique event.

The objective of this conference is to learn about hospital fires from the perspective of fire experts and hospital leaders. Actual hospital fires and how they were handled will be discussed in order to develop ideas for best practices for hospital fire prevention and response.

LAYING THE FOUNDATION FOR THE WORLD'S LARGEST UNDERGROUND HOSPITAL

Seven thousand cubic meters of concrete were poured this week to form the base of the world's largest underground hospital. For 36 hours running, shifts of 70 workers and 80 cement mixers worked to lay the foundation of the emergency facility, which is designed to withstand conventional, chemical, and biological attacks

The night of Saturday October 9, 2010 marked the start of a crucial phase of construction at the Rambam Health Care Campus in Haifa, Israel. More than 70 workers at the site began to pour roughly 7,000 cubic meters of concrete, which will form the concrete base of Rambam's protected emergency underground hospital - the largest facility of its type in the world - and underground parking lot. The pouring, which continued for more than 36 hours straight, involved 80 cement mixers that completed 1,000 rounds of mixing. To meet the deadline, four Haifa area concrete plants supplied materials around the clock.

According to Rambam Department of Engineering Director Aryeh Berkovitz, for two days following this effort, no concrete was poured in central to northern Israel. This is due to the fact that all related facilities, tools and personnel in the region were involved in the huge Rambam project.

RHCC Director Prof. Rafi Beyar commented, "This is a historic moment, not only for Rambam, but for the entire State of Israel. For a period of two years, we have coped with unexpected, difficult and weighty logistic problems regarding this construction. We overcame the obstacles, and with the help of our friends - donors and Ministry of Health officials - we are on the right path."

This project is slated for completion by May 2012, at which time the three-floor parking lot will provide much-needed parking space for some 1,500 Rambam workers and visitors. In times of emergency, the lot can be transformed at short notice into a 2,000-bed hospital that is secure from conventional, chemical and biological weapons.

Not only underground, the emergency hospital will also sit eight meters below sea level. Designed to be self-sufficient, the hospital will be able to generate its own power and can store enough oxygen, drinking water and medical supplies for up to three days. Due to its special location, the facility's construction has demanded unusual measures. Pumps have operated
throughout construction - and will continue to the project's end - 24 hours a day, moving millions of cubic meters of brackish groundwater to the sea, enabling the workers to carry out their mission.

The next major step in the Rambam complex construction - the concrete pouring for the foundation of the Ruth Rappaport Children's Hospital - will begin on October 30, 2010. After that, work will begin on the new oncology and cardiology hospitals.

"Work is proceeding in a very ordered and professional manner," says Aryeh Berkovitz, who reports that all construction is proceeding according to plan. Due to the project's enormous scope and special nature, it has been making headlines. The recent cement pouring has received coverage on all major radio stations, TV channels and in the printed and online media throughout Israel.

National Commission on Children and Disasters


On Wednesday, October 6, 2010 the National Commission on Children and Disasters (NCCD) delivered its landmark 2010 Report to the White House and Congress. The report contains more than 100 recommended actions for Federal and non-Federal partners.

The report is available at http://www.childrenanddisasters.acf.hhs.gov/.

Children represent 25% of our population. Their unique needs must be addressed throughout disaster management laws, policies, programs and planning. While the report cites areas of progress, there are still significant gaps:

* Only 6% of hospital emergency rooms and less than 20% of ambulances carry essential supplies and equipment for children

* Only 12 states meet basic emergency preparedness standards for children, such as requiring schools to have a multi-hazard preparedness plan, and child-care facilities to have a plan that addresses evacuation, reunification and accommodating children with disabilities or other special needs

* We lack a cohesive national strategy to support the critical needs of children before, during and after disasters

The work of the National Commission on Children and Disasters is far from over. The Commission looks forward to continuing to work with all interested parties toward a common goal of turning the Commission's recommendations into reality.

Fwd: Health Leaders Urge Action to Make Hospitals Safe in Disasters

In Latin America and the Caribbean, health facilities continue to be built in high-risk areas without proper protective measures

Washington, D.C., Sept. 29, 2010 (PAHO) — Ministers of health from throughout the Americas called today for new efforts to make hospitals safer in disasters so they can continue providing health services at times of urgent need.

The health leaders, meeting at the Pan American Health Organization (PAHO) in Washington, D.C., endorsed a new regional plan that includes planning, investments, and oversight systems to ensure that new hospitals and other health facilities are built and equipped to be disaster-resilient, and that older facilities are upgraded to be able to remain operational.

Reports from PAHO member countries indicate that 67 percent of existing health facilities in the Americas are located in disaster risk areas, and that new health facilities continue to be built in these areas. This is because most countries lack up-to-date standards, oversight mechanisms, and the administrative power to guarantee the safety and continuous operation of health facilities.

Data are not available on how many facilities in high-risk areas are vulnerable to disasters. However, evaluations of 327 hospitals in 17 countries using the "Hospital Safety Index" found that only 36 percent of assessed hospitals had a high probability of remaining functional following a disaster. Sixteen percent required urgent measures because they were considered unlikely to be able to protect the lives of patients and health personnel in the event of a disaster. 

When a hospital is rendered inoperative during a disaster, the loss of emergency services makes it more difficult to save lives. Moreover, for every failed hospital, on average 200,000 people are deprived of health care for months and sometimes years.

In Haiti's earthquake in January of this year, 22 hospitals were seriously damaged and eight were destroyed in the three most-affected regions. Field hospitals—which took days or weeks to set up—provided limited health services to survivors needing medical attention.

In Chile's February 27 earthquake, 79 out of 130 hospitals in the affected area were damaged, for a total loss of some 4,700 hospital beds. Most hospitals that suffered serious damage were older structures that had not been upgraded to reduce their vulnerabilities to disasters. Many of these were near, and in some cases just meters away from, new hospitals that had been built to "safe hospital" standards and suffered little or no damage.

The action plan approved today calls on all PAHO member countries to:

  • Establish national "safe hospitals" programs that set priorities for investments and include the participation of all concerned institutions and sectors.
  • Develop systems to track new construction, repairs and improvements in health facilities, and establish transparency mechanisms for financing and execution of health infrastructure projects.
  • Create systems for supervision and control, and develop a cadre of experts on the structural, nonstructural, and functional safety of health facilities to staff them.
  • Develop a legal framework to ensure that new facilities and support services have the basic conditions to keep operating in the event of disasters.
  • Develop technical guidelines for locating health facilities and for damage mitigation measures, to be included as terms of reference in bidding tenders for design, construction, and operation. Develop safety standards to be incorporated into health facility financing agreements.
  • Improve the safety of existing health facilities, giving priority to those that are an essential part of the health services network during emergencies and disasters.

In approving the new regional action plan, ministers of health noted growing public demand and political will for safe hospitals, illustrated by a number of national and international commitments in this area. Colombia, Ecuador, Mexico and Peru, among others, have adopted national safe hospital plans, while other countries are drafting or updating legislation on the design and construction of health facilities to withstand disasters and the operation of these facilities in disaster situations. At least 24 countries and territories in the Americas have trained evaluation teams in the use of the Hospital Safety Index, and hundreds of hospitals have been assessed. 

The PAHO Directing Council brings together ministers of health and other high-level delegates from throughout the Americas each year to set priorities for Pan-American cooperation in health and to guide PAHO's technical cooperation programs in its Member States. 

PAHO was established in 1902 and is the world's oldest public health organization. It works with all the countries of the Americas to improve the health and quality of life of the people of the Americas and serves as the Regional Office for the Americas of the World Health Organization (WHO).
